    This Privacy Policy explains how University Physical Medicine (“we,” “us,” or “the practice”) collects, uses, and protects information when you visit chiropractictallahassee.com. It describes our use of cookies and similar technologies and the choices you have.

    This website notice is separate from our HIPAA Notice of Privacy Practices, which governs protected health information created or received in the course of treatment.

    Information we collect

    • Information you provide. When you submit a contact or appointment-request form, request a callback, or use our website chat assistant, we collect the details you enter — typically your name, phone number, email address, the service you’re interested in, and any message you write. Please do not enter detailed medical history in free-text fields.
    • Automatically collected information. Like most websites, our servers and (with your permission) analytics tools may record technical data such as IP address, browser type, device, pages viewed, and referring page.

    Cookies and your choices

    We use a small number of essential technologies that are always on because the site cannot function without them (for example, remembering your cookie choice and operating our contact forms and chat). These do not track you for advertising.

    We use analytics and advertising cookies and tags — currently Google Analytics 4 and Google Ads conversion tracking{{#IF_META}} and the Meta (Facebook) Pixel{{/IF_META}}only after you opt in through our cookie banner. Until you choose “Accept all,” these tags do not load and set no cookies. If you choose “Reject non-essential,” we do not load them and we clear any related cookies.     We operate Google tags using Google Consent Mode, which defaults advertising and analytics storage to denied until you consent. We do not load Google Analytics or Google Ads on pages about specific health services, conditions, intake paperwork, appointment booking, or contact — and we strip service- and condition-specific details from any conversion measurement. Google does not sign business-associate agreements for these products, so we deliberately keep health-identifying information away from them.

    How we use information

    3. To operate, secure, and improve the website
    4. With your consent, to measure website traffic and the performance of our advertising

    We do not sell your personal information.

    How information is shared (service providers)

    We use trusted service providers to operate the website and manage inquiries. Information you submit through our forms and chat is processed and stored using:

    • Our website & lead platform (Supabase / Revive OS). Form and chat submissions are transmitted to and stored on our agency platform’s secure backend to deliver them to our team.
    • Our patient-relationship system (GoHighLevel). Contact details from your inquiry are added to our CRM so we can follow up by call, text, or email (you control SMS follow-up through the consent checkbox on the form and may reply STOP at any time).
    • Analytics & advertising (Google; {{#IF_META}}Meta{{/IF_META}}), only with your consent, as described above.
